Aplastic anemia

Pathogenesis of aplastic anemia

Infection of human body with the environmental pollution or drug abuse can bring defects to hematopoietic stem cells and abnormalities to blood cells; damage blood microcirculation environment, cause abnormality of negative regulation of hematopoiesis and dysfunction of immunity. In the end it will lead to hematopoietic disorders. Hemopoietic function of bone marrow depends on a sufficient number of normal stem cells, stem cells must be able to keep in a constant quantity by repeat self-proliferation, and on the other hand, they must be able to form large number of mature blood cells by differentiation as erythrocyte line, leukocyte line and megakaryocytic line. Brief introduction

Aplastic anemia belongs to multipotent stem cells disorders. In clinical there are often severe anemia, infection and bleeding. The majority of patients are young adults, and the male are more than the female.

Causes

In many cases, the etiology is impossible to determine, which is called primary aplastic anemia. But aplastic anemia is sometimes associated with exposure of bone marrow to toxin from chemical, physical and biological factors, which is called secondary aplastic anemia. The most common reason is poisoning or allergies after contact with chemicals in pharmaceutical industry or life, followed by the various forms of ionizing radiation, the rare cases are viral infection and immune response.

Signs and symptoms

The main clinical manifestations are progressive anemia, bleeding and infection, severity of which has relationship with the extent of reduction of the blood cells and the speed of development. Other symptoms include fatigue, weakness, anaemia symptoms such as pale skin and mucosa, petechia and ecchymosis of skin and mucousa, gingival bleeding, epistaxis, menorrhagia in female patients, infection symptoms in oral, around anal, skin and upper respiratory tract. There is no swollen of lymph nodes and spleen in general. Repeated infections and long-term repeated blood transfusion will also enable mildly enlarged spleen.

1. Acute type:

Most cases are with urgent onset and severe symptoms. Typical early symptoms are infection and bleeding. Other common symptoms include high fever, chills, sweating, oral or throat ulcers, skin infections, and pneumonia, skin petechia and ecchymosis, epistaxis, gingival bleeding, gastrointestinal bleeding, and menorrhagia in female patients. Patients can die from septicemia in severe cases. Such cases are severe with short course.

2. Chronic type:

Most cases are with chronic onset, the main symptoms include fatigue, short breath after tiredness, palpitations, dizziness, and pale. It is mild if there is bleeding and visceral bleeding is rare. Infection and fever are mild and appear in relative late stage, which are easy to be controlled after treatment. Liver, spleen and lymph nodes are not swollen, but sometimes there is mildly enlarged spleen in advanced cases. The course is relatively long and patients can survive for many years. Conditions can gradually improve, some of which are even close to full recovery. Some cases change into acute type.

Examination

1. Blood test:

The most important characteristic is pancytopenia, but reduction degree of red blood cells, platelets and tablets are varied.

2. Bone marrow aspiration:

In acute cases, there are very small amount of marrow particles and fat drops increase obviously. Endoscopic examination shows reduced or severely reduced hematopoiesis of bone marrow. Purpura

The reasons for bleeding in purpura:

(1) Vascular system diseases: because there are pathological changes happened to vascular system, such as injury to the vessel wall or increased permeability and brittle of the vessel wall, red blood cells will leak out, which will form purpura. Vascular injury may be caused by bacterial toxins, chemical drugs, lack of vitamin and so on, such as simple purpura, allergic purpura and idiopathic thrombocytopenic purpura.

(2) Hematological system diseases: bleeding caused by clotting dysfunction of hematological system. Such as thrombocytopenic purpura, hemophilia, fibrinogenopenic purpura, prothrombinopenic purpura caused by liver diseases and purpura caused by excessive use of anticoagulants. These diseases can not only lead to bleeding of the skin, but also trigger bleeding of other tissues and organs. Therefore, we should not treat purpura lightly.

According to clinical symptoms

(1) Simple purpura: There are no systemic symptoms in general; there could be scattered bright red needle-sized petechia around two lower limbs, which do not fade when pressed. Petechia will fade and dissipate seven days later, but new ones will appear.

(2) Idiopathic thrombocytopenic purpura: It can be divided into acute and chronic type. Acute type is rare, which can occur in children and there is always infection before onset. it is characterized by sudden onset, fever, extensive skin bleeding, may accompanied with large ecchymosis and hematoma, and there are always bleeding of mucosa and genitourinary system, platelet reduced significantly to lower than 5000 /mm3. Chronic type is a common type, which always happens to young women with a slow onset. It is characterized by recurrent or persistent bleeding not only in the skin, there might be serious bleeding in mucous membrane and internal organs.

(3) Purpura rheumatica: There may be slightly uplifted rice-sized ecchymosis, petechia, wheal or edema erythema. At the same time there might be painful swelling of joints, fever, weakness and other systemic symptoms.

(4) Henoch-Schonlein purpura: Skin changes of Henoch-Schonlein purpura are like purpura rheumatica, other symptoms include abdominal pain, vomiting, diarrhea, tenesmus, and hemafecia. Severe cases can be complicated with intussusception.

Purpura in children

Purpura is a common haemorrhagic disease in children. In clinical, it is featured by ecchymosis and petechia under the skin and mucous membrane, which don't fade when pressed. It always happens in the school-age children, including thrombocytopenic purpura and anaphylactoid purpura in Western medicine.

Diagnosis

Acute onset. Repeated purpura can occur in skin and mucous membrane around the body. Rashes are diverse, there may be needle-like small ones, and the big ones can form ecchymosis or hematoma, or even integrate together. Rashes are pale red or bright red and does not fade under pressure, which will turn into lilac-like color and gradually dissipate. Acute onset often accompanied with epistaxis, gingival bleeding, hemafecia, hematuria, vomiting, abdominal pain and painful swelling of joints. It belongs to blood disease, which should be differentiated with macula in the warm disease. The occurrence of warm disease always has relationship with pathogenic factor in four seasons and always has infectious. It proceeds with high fever and headache, and followed by macula. It is acute onset with rapid change. There are coma and convulsions in severe cases.

Laboratory test:

If there are decreased number of platelets, extended bleeding time, normal clotting time, poor clot retraction, positive anti-platelet antibody and positive tourniquet test, which suggest it thrombocytopenic purpura; if it is normal of platelet count, bleeding time, clotting time and clot retraction, which suggest it anaphylactoid purpura.

Hemophilia

Hemophilia is a group of hereditary genetic disorders that impair the body's ability to control blood clotting or coagulation. It includes:

hemophilia A is absence of clotting Factor VIII (antihemophilic factor);

Hematoma can be absorbed completely within days and weeks in primary cases, the pain will disappear and function will return to normal. If there is repeated joint hemorrhage, hematoma can't be absorbed completely, which will lead to chronic inflammation in joints, thickening of synovium. A few months or years later, the synovium and bone will be destructed and joint fibrosis appears, which will lead to ankylosis, joint deformity and loss of function.

Hemophilia is hereditary disease; there is no radical solution to it until now. The effective way is to infuse patients with concentrated clotting factors, which is known as the clotting factor therapy. However, effect of clotting factor transfused into the body can not last for a long time due to limited half-life. The half-life of factor VIII is 8-12 hours and that of factor IX is 18 -24 hours.
